I, the undersigned, hereby, agree to ensure that my Child/children, or any other child/children in my care, is/are supervised at all times, and do not behave in any manner so as to cause any danger or harm to themselves or to others. In the event of an accident or a mishap or otherwise, I will not hold the owners, and/or the Management and/or any member of the Funky Monkeys staff liable for any injury or accident caused to any child/children or myself during my visit.
